Patient Information
Preparing for your visit

If you are a new patient to our practice it is likely that you will have your eyes dilated on your first visit.  Dilating drops will temporarily blur your vision and last for a few hours.  We recommend a driver.  Whether you choose to bring a driver or drive yourself you may want to bring a pair of sunglasses due to the fact you may be light sensitive until the dilation wears off.

There are some items you should bring with you below:

If you have access to a printer you can print the new patient forms located under the forms tab, fill them out in advance and bring with you to your appointment.
  • Insurance Cards
  • Prescription Coverage Card
  • List of medications (all Medications including eye drops & vitamins)
  • Most current eyeglasses Distance & Reading
  • Last office note from your optometrist including your most recent eyeglass prescription.

These records can be faxed to our office at 518-828-6734.

Last office note from your primary care physician including list of current medications and Your last A1C blood test result if you are a diabetic. These records can be faxed to 518-828-6734. 

If you have an appointment due to cataracts, you need to use artificial tears, 1 drop in each eye 3 times a day for 1 week prior to your appointment.  You can purchase them at any pharmacy.  You must also stop wearing contact lenses for 3 weeks prior to the appointment to get accurate measurements.
